COURT-RELATED FEES:

• Subpoena: $500 per day for each provider (retainer required)
• Deposition: $500 per day for each provider (retainer required)
• Administrative time: $160 per hour, in 15-minute increments

Notes:
• The $500 retainer payment for all Subpoenas and Depositions must be paid no later than 7 days prior to appearance.
• No refunds or partial payments are provided.
• Administrative time applies to requests for Summary of Service, correspondence, and all other court-related requests for information.
